Support Services

HIV/AIDS, Mental Health, Substance Abuse Services

Starting as early as 2008, AAPCI began to move toward offering a complete range of services for individuals infected/affected by HIV/AIDS, substance abuse, chronic homelessness and mental health disorders, including case management, homeless prevention, training and education, counseling and supportive services, advocacy, and permanent supportive housing.

Divided into four main categories, our services address the full array of challenges faced by special-needs individuals, those living with mental health disorders, and those struggling with substance abuse sobriety. Our programs provide treatment adherence, access to medical care, mental health services and nutritional support services. We also offer assistance with legal issues like living wills, standby guardianship, and health care proxies. Together, these services enable our clients remain healthy and achieve self-sufficiency.

Housing Services

AAPCI recognizes that individuals living with HIV/AIDS, substance abuse or mental health disorders cannot always maintain healthy lifestyles without a safe and affordable place to call home. Starting with this vision, we offer permanent supportive housing to clients.

Staffing Pattern & Social Support Services

Offered in connection with Special Needs housing:

All support services will be provided free of charge by an interdisciplinary team of English and bilingual staff and through linkage agreements with other community-based service providers.

Support services will include on-site staff and/or linkages to off-site professionals who will provide:

  • Case Management services
  • Individual & Group Counseling sessions,
  • Intensive Case Management (ICM) services,
  • Assistance with obtaining and maintaining Public Benefits,
  • Health and Mental health services,
  • Substance abuse treatment services,
  • Medication management services,
  • Alternate Housing search/placement assistance,
  • Vocational enrollment,
  • Job search/placement assistance,
  • Basic computer skills classes,
  • Information and referral services,
  • Peer support groups, on site self-help services,
  • Independent living skills services,
  • Homebound healthcare services,
  • Nutrition and nutritional plan services,
  • Healthcare referrals and follow-up services,
  • Educational classes on issues such as breast cancer, nutrition, HIV/AIDS, and overall family health,
  • Cultural and recreational program services,
  • Linkage and referrals to a wide array of community-based service providers including:
    • NYC Department of Homeless Services (DHS),
    • NYC Health and Hospitals Corporation (HHAC),
    • NYC Department of Health and Mental Hygiene (DOHMH),
    • NYC HIV/AIDS Services Administration (HASA)

Formal and informal relationships with a wide array of community-based service providers.
